In this role, you'll be working in conjunction with a multi-disciplinary team in the delivery of home-based Allied Health Services in a customer-led organisation.
This includes providing clients and caregivers with available options and solutions within the assessment brief and funding guidelines.
A week in the life of your new role
Providing high-quality Allied Health assessment, intervention and care for clients within the context of primary health care in a professional, confidential and culturally safe manner, in keeping with accepted best practice standards
Conducting home-based Allied Health assessments including all areas of activities of daily living, mobility and home safety
Ensuring integrated service delivery and follow up, coordinating and communicating as required with other relevant internal, visiting and external providers
Providing follow-up monitoring and care as directed through health assessments
Incorporating a holistic approach to health care that incorporates health promotion activities according to community and organisational priorities
Working closely with the local community teams and key stakeholders to support regular tracking of progress in monitoring client progress and care, ensuring feedback is provided to staff supporting implementation of changes in response to identified gaps
Qualifications and other prerequisites required
Prior Community Experience highly desirable
Cert IV in Allied Health or equivalent
Ability to provide high quality treatment and intervention
Police Check (dated within the last 3 years)
Willing to obtain annual flu vaccinations
Evidence of Covid 19 vaccinations
Own vehicle and NSW licence
